Insurance Information

At Tuttle and Sawmill Eye Associates, we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures.  

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

Our Doctors are participating in-network providers for most of the major medical and vision insurers in the area.  Please bring your insurance information with you to your visit so that we can expedite reimbursement.  Co-payments and payments for non-covered services and materials are is due in full at the time of your visit.

Medicare Patients: Medicare pays for 80% of your care after your deductible has been met. You are responsible for your deductible, out-of-network co-insurance and non-covered services (refraction and digital retinal photos). 

Commercial Insurance: As a courtesy, we will file your insurance claims for you. Upon receipt of an insurance payment, any balance due will be billed to you and is your financial responsibility.   

Failure of your insurance to pay for services and products rendered does not excuse your financial responsibility.
